Pragmatic Free Trial Meta
Pragmatic Free Trail Meta is an open data platform that facilitates research into pragmatic trials. It gathers and distributes clean trial data, ratings and evaluations using PRECIS-2. This allows for diverse meta-epidemiological studies to examine the effect of treatment across trials of different levels of pragmatism.
Background
Pragmatic studies provide real-world evidence that can be used to make clinical decisions. However, More suggestions of the term "pragmatic" is not consistent and its definition and assessment requires clarification. The purpose of pragmatic trials is to guide clinical practice and policy decisions, rather than confirm a physiological or clinical hypothesis. A pragmatic trial should aim to be as close as it is to real-world clinical practices which include the recruiting participants, setting, design, delivery and implementation of interventions, determination and analysis results, as well as primary analyses. This is a major difference from explanatory trials (as described by Schwartz and Lellouch1) which are designed to provide more thorough proof of an idea.
Studies that are truly pragmatic should not attempt to blind participants or healthcare professionals as this could result in distortions in estimates of treatment effects. Pragmatic trials will also recruit patients from various health care settings to ensure that their results can be applied to the real world.
Additionally, clinical trials should focus on outcomes that matter to patients, like the quality of life and functional recovery. This is especially important in trials that involve invasive procedures or those with potentially dangerous adverse events. The CRASH trial29 compared a two-page report with an electronic monitoring system for hospitalized patients with chronic cardiac failure. The trial with a catheter, however was based on symptomatic catheter-related urinary tract infections as its primary outcome.
In addition to these characteristics, pragmatic trials should minimize trial procedures and data-collection requirements to cut down on costs and time commitments. Additionally, pragmatic trials should aim to make their results as relevant to real-world clinical practices as they can. This can be achieved by ensuring that their analysis is based on the intention to treat approach (as defined in CONSORT extensions).
Despite these guidelines however, a large number of RCTs with features that defy the concept of pragmatism have been mislabeled as pragmatic and published in journals of all types. This could lead to false claims about pragmatism, and the usage of the term should be standardised. The development of a PRECIS-2 tool that provides an objective, standardized evaluation of pragmatic aspects is a good start.
Methods
In a pragmatic trial the goal is to inform policy or clinical decisions by demonstrating how the intervention can be incorporated into real-world routine care. Explanatory trials test hypotheses regarding the causal-effect relationship in idealized environments. Therefore, pragmatic trials might be less reliable than explanatory trials and might be more susceptible to bias in their design, conduct and analysis. Despite these limitations, pragmatic trials can contribute valuable information to decisions in the context of healthcare.
The PRECIS-2 tool scores an RCT on 9 domains, with scores ranging between 1 and 5 (very pragmatic). In this study, the recruitment, organization, flexibility in delivery, flexible adherence and follow-up domains received high scores, however, the primary outcome and the method for missing data were below the practical limit. This suggests that it is possible to design a trial that has high-quality pragmatic features, without damaging the quality of its results.
However, it is difficult to judge the degree of pragmatism a trial is since the pragmatism score is not a binary characteristic; certain aspects of a trial may be more pragmatic than others. Moreover, protocol or logistic modifications made during the trial may alter its score in pragmatism. In addition 36% of the 89 pragmatic trials discovered by Koppenaal and co. were placebo-controlled, or conducted prior to licensing, and the majority were single-center. Thus, they are not quite as typical and can only be described as pragmatic when their sponsors are accepting of the lack of blinding in such trials.

A common feature of pragmatic research is that researchers try to make their findings more meaningful by studying subgroups within the trial sample. This can lead to unbalanced analyses with less statistical power. This increases the chance of omitting or misinterpreting differences in the primary outcomes. In the instance of the pragmatic trials included in this meta-analysis, this was a serious issue since the secondary outcomes were not adjusted for the differences in the baseline covariates.
Furthermore, pragmatic trials can also be a challenge in the collection and interpretation of safety data. This is due to the fact that adverse events are usually self-reported, and are prone to delays, inaccuracies or coding errors. It is therefore crucial to improve the quality of outcomes assessment in these trials, in particular by using national registries rather than relying on participants to report adverse events on the trial's database.
Results
While the definition of pragmatism does not mean that trials must be 100 percent pragmatic, there are advantages to incorporating pragmatic components into clinical trials. These include:
Increased sensitivity to real-world issues which reduces the size of studies and their costs as well as allowing trial results to be faster transferred into real-world clinical practice (by including patients who are routinely treated). However, pragmatic trials have disadvantages. For example, the right type of heterogeneity can help the trial to apply its findings to a variety of patients and settings; however the wrong kind of heterogeneity could reduce assay sensitivity and therefore decrease the ability of a study to detect minor treatment effects.
A variety of studies have attempted to classify pragmatic trials using a variety of definitions and scoring methods. Schwartz and Lellouch1 developed a framework to differentiate between explanation studies that prove a physiological hypothesis or clinical hypothesis, and pragmatic studies that inform the choice for appropriate therapies in clinical practice. Their framework comprised nine domains that were scored on a scale of 1 to 5 with 1 being more informative and 5 suggesting more pragmatic. The domains included recruitment, setting up, delivery of intervention, flex adherence and primary analysis.
The original PRECIS tool3 included similar domains and an assessment scale ranging from 1 to 5. Koppenaal and colleagues10 developed an adaptation to this assessment called the Pragmascope that was easier to use in systematic reviews. They discovered that pragmatic systematic reviews had higher average scores across all domains, but lower scores in the primary analysis domain.
This distinction in the primary analysis domains can be due to the way in which most pragmatic trials analyse data. Certain explanatory trials however don't. The overall score for systematic reviews that were pragmatic was lower when the areas of organisation, flexible delivery and following-up were combined.
It is important to remember that a pragmatic trial doesn't necessarily mean a low-quality trial, and there is a growing number of clinical trials (as defined by MEDLINE search, but it is neither sensitive nor specific) that employ the term 'pragmatic' in their title or abstract. The use of these terms in abstracts and titles may suggest a greater awareness of the importance of pragmatism but it is unclear whether this is reflected in the content of the articles.

In recent years, pragmatic trials have been becoming more popular in research as the value of real-world evidence is increasingly recognized. They are randomized studies that compare real-world alternatives to experimental treatments in development. The authors of the Pragmatic Free Trial Meta identified RCTs published from 2022 to 2022 that self-described as pragmatic. They assessed pragmatism by using the PRECIS-2 tool that includes the domains eligibility criteria as well as recruitment, flexibility in intervention adherence, and follow-up. They discovered that 14 of these trials scored highly or pragmatic practical (i.e. scores of 5 or more) in one or more of these domains and that the majority were single-center.
Trials with a high pragmatism rating tend to have broader eligibility criteria than traditional RCTs which have very specific criteria that are not likely to be present in clinical practice, and they comprise patients from a wide range of hospitals. The authors claim that these characteristics could make pragmatic trials more effective and applicable to everyday clinical practice, however they don't necessarily mean that a trial conducted in a pragmatic manner is free of bias. The pragmatism principle is not a fixed attribute the test that does not possess all the characteristics of an explanatory study can still produce valid and useful outcomes.
